Why month 1 looks broken

At 180 lbs the first four weeks average only 3.6 lbs — about 2.0% of your weight. That is not the drug failing. The starter dose is a tolerance ramp, and the steep part of the curve does not arrive until you are on a full treatment dose. Here is the schedule the trial used:

  1. 0.25 mg (weeks 1-4, starter dose - not a treatment dose)
  2. 0.5 mg (weeks 5-8)
  3. 1.0 mg (weeks 9-12)
  4. 1.7 mg (weeks 13-16)
  5. 2.4 mg (week 17 onward)

Wegovy is semaglutide, the same molecule sold as Ozempic for type 2 diabetes, but dosed higher (2.4 mg vs 1.0-2.0 mg).

Questions people actually ask

How much weight will I lose in the first month on Wegovy at 180 lbs?

About 3.6 lbs, taking 180 lbs to roughly 176 lbs. Month 1 is the flattest stretch of the whole curve because you are still on the starter dose, which exists to let your gut adjust, not to drive loss. Anything from almost nothing up to about 6 lbs sits inside the normal spread for month 1.

Is 27 lbs guaranteed?

No. These are trial averages. In STEP 1 the spread was wide: a large minority lost far more than average, and a real share of people lost under 5% of their weight. Dose, how long you stay on it, and whether you hit a protein target all move your own number.

What is the fastest part of the curve?

Roughly weeks 12 to 36, once you are on a full treatment dose. In this scenario the average runs near 0.50% of body weight per week through the early titration and then steepens before flattening out toward week 68.
