A GLP-1 costs as little as $25 a month with commercial coverage and a savings card, and $149 to $449 a month for a brand-name drug without insurance: Zepbound vials $299–449 by dose through LillyDirect, Wegovy pens $349 through NovoCare Pharmacy. Compounded versions cost less and have no FDA approval. Over a year, the first year from your first dose runs from about $325 with commercial coverage and a savings card, through $828–$4,788 for compounded semaglutide or tirzepatide (a one-year prepaid plan to month to month), to $4,237–$5,637 for brand-name self-pay, titration fills included. Per pound lost, for the brand-name drugs at the dose their trial tested: ~$7–10 with a savings card and $126–149 self-pay, from each dose's week-52 loss in SURMOUNT-1 and STEP 1, whose participants started around 230 lb. Compounded products were not in those trials, so they are compared on price only: from $69 a month on a one-year plan (billed $828/yr upfront, non-refundable), or $79 a month for the first 4 weeks. Zepbound without insurance: through LillyDirect Self Pay, vials cost $299 a month at 2.5 mg, $399 at 5 mg and $449 at 7.5 mg and above, when each refill comes within 45 days of the last delivery; after that, $499 at 7.5 mg and $699 at 10 to 15 mg. Last known price, Oct 7, 2026 — may have changed · confirm on LillyDirect
First month against later months: the first month is the 2.5 mg starting dose, the cheapest fill. The label raises the dose in 2.5 mg steps after at least four weeks on each, so month two at 5 mg costs $399, and a maintenance dose of 7.5 mg or more costs $449 a month. With commercial coverage of Zepbound, the savings card brings any dose to as little as $25 a month. Last known price, Oct 7, 2026 — may have changed · confirm on LillyDirect
Compounded tirzepatide is made by a compounding pharmacy, not Lilly, has no FDA approval, and starts at $119/mo on a one-year plan at the providers we track. First month against later months, by provider: compounded tirzepatide price. Price checked Oct 26, 2026 · joinem.co
Per pound lost, only the brand-name drug has a trial result: the route table.
Wegovy without insurance: through NovoCare Pharmacy, pens cost $349 a month from 1 mg to 2.4 mg and $399 for the 7.2 mg pen. The Wegovy pill is $149 at 1.5 mg, $199 at 4 mg and $299 at 9 mg and above. Price checked Oct 26, 2026 · novocare.com
First month against later months: the pens start at 0.25 mg and step up every four weeks; a new patient's two starter doses cost $199 a month through December 31, 2026, then $349 from 1 mg up. The pill's price rises with its strength the same way. With commercial coverage of Wegovy, the savings card brings it to as little as $25 a month. Ozempic is the same molecule approved for type 2 diabetes, not weight management. Wegovy's cost per pound lost is in the route table.
Two separate hurdles. First, the FDA label: a fixed rule about BMI and weight-related conditions. Second, your plan's coverage criteria, which are stricter and vary. Check the label first; everything below it assumes you clear it.
The label is the floor. Almost every plan that covers these drugs adds a prior authorization, with criteria set by your employer or insurer, not the manufacturer. What they commonly ask for:
A documented weight history, not just today's weight. Plans often want the BMI threshold met on a dated clinical record, sometimes more than one, sometimes going back six or twelve months.
Evidence you tried something else first. This catches most people. It can mean a documented three to six months of a supervised diet and exercise program; self-reported attempts usually do not count. If you have worked with a dietitian, a hospital program, or a previous weight medication, get those records.
The weight-related condition spelled out in the chart. For sleep apnea, a plan generally wants the sleep study, not a mention. For cardiovascular disease, it wants the diagnosis coded.
Re-authorization. Many plans approve six months at a time and require a minimum loss, often around 5% of starting weight, to renew. Know this on day one: your early weigh-ins matter for paperwork too.
Denials can involve missing documents, unmet criteria, or plan exclusions. Choose resubmission, reconsideration, peer-to-peer review, or appeal based on the denial reason.
Find out the actual reason. Ask for the denial in writing, with the specific criterion not met.
Ask the prescriber's office about a peer-to-peer review. Your prescriber talks directly to the plan's reviewing physician. Process and clinic fees vary.
File the formal appeal, then the external review. Plans must give you an appeals path and, in most cases, an independent external review once internal appeals are exhausted. Deadlines are short, so start the day the denial arrives.
Ask about a formulary exception if the plan covers one GLP-1 but not yours, and whether switching to the covered one is clinically reasonable. Often it is, and it is faster than fighting.
The full appeal playbook, with deadlines, is on what to do when insurance won't cover it, and the free appeal letter builder there sorts your denial and drafts the letters. If none of it works, self-pay channels cost less than list price. The cost calculator shows what a year runs on each channel, and which drug to ask for covers how coverage should shape the choice.
Weight history with dates; blood pressure, lipid panel and A1c if you have them; a sleep study if one exists. A written list of what you have tried, with rough dates and duration. Your insurance card and, ideally, the formulary answer from calling the number on the back.
Also useful: any weight-related conditions in the chart (high blood pressure, sleep apnea, prediabetes, high cholesterol, PCOS); family history of thyroid cancer and any personal history of pancreatitis or gallbladder problems; whether you are pregnant, breastfeeding or planning to be; and every medication you take, especially insulin, sulfonylureas and anything for thyroid.
No prescriber yet? Every provider we track lists the licensed US platforms prescribing the brand-name versions; several handle the prior-authorization paperwork for you. Then run the curve to see what a normal month 1, 3 and 6 look like.
Pick the drug, the maintenance dose and the way you pay. The calculator counts every fill from your first dose, the lower-priced titration fills included, and divides the total by the average loss that same dose's trial reported at that point. It uses the same numbers as the route table below, so the same choices give the same answer in both.

Because the drugs do not lose the same amount. In SURMOUNT-5, the head-to-head trial, tirzepatide averaged −20.2% of body weight at 72 weeks against −13.7% for semaglutide, so a drug that costs more per month can finish cheaper per pound. Loss figures in the calculator are the same trial means as the curve calculator: STEP 1 for Wegovy, SURMOUNT-1 for Zepbound, OASIS 4 for the Wegovy pill. All the trials paired the drug with diet and activity counselling, and individual results spread widely.
Every route this site tracks, cheapest first year first, in the billing view you choose: month to month (the default) or Annual prepay, which prices compounded rows on their one-year plan, billed upfront. A brand row is one maintenance dose. Its first year counts every fill from your first dose, the lower-priced titration fills included; a fill lasts 28 days, so a 52-week year is 13 fills, not twelve calendar months. The ongoing column is a year at the maintenance dose. Weight loss is that same dose's trial average at week 52, estimated from the trial curve (the trials report week 72 and week 68), applied to a 230 lb start, close to the mean starting weight in both trials: Zepbound 5 mg 14.8% (34 lb), 10 mg 18.5% (43 lb) and 15 mg 19.5% (45 lb) in SURMOUNT-1; Wegovy 2.4 mg 14.3% (33 lb) in STEP 1. Those figures, and the cost per pound worked from them, apply to the brand-name rows only: no compounded product was tested in either trial, so compounded rows show no weight loss and no cost per pound. Highlighted rows are the lowest month-to-month compounded price for each drug.

Each price shows the date its provider's own page last confirmed it, with a link to that page; a price that could not be re-confirmed is shown as the last known price. We earn a commission from some providers; it never changes the order. How we make money
A promotional first-fill price with no published renewal price gets no yearly total ("Not confirmed") and is not ranked on one. A one-year plan that delivers fewer than 52 weeks of medication shows its plan total and the weeks it covers; its 52-week total is not confirmed, and it is ordered by its price per week of medication. Uses current prices and offer terms; future prices are not guaranteed.
Set a calendar reminder ~35 days after each LillyDirect delivery — missing the 45-day window raises 10–15 mg vials from $449 to $699. Last known price, Oct 7, 2026 — may have changed · confirm on LillyDirect
Each price shows the date it was last confirmed. One-year plans are charged upfront and are often non-refundable: switch to Annual prepay to see them with their terms. Compounded products were not tested in either trial, and brand-name results do not carry over to them, so they are compared on price only. For your own weight and months on a brand-name drug, use the cost per pound calculator above.
Manufacturer self-pay does not touch your deductible. Cash through NovoCare or LillyDirect is often the cheapest monthly number, but none of it counts toward your deductible or out-of-pocket maximum. High-deductible plan and other medical costs expected this year? Insurance can be cheaper overall, even if the monthly figure looks worse.
Medicare and Medicaid enrollees generally can't use manufacturer savings cards. Cash-pay program eligibility varies by manufacturer — check the current terms. The self-pay prices are a different program from the cards: Novo Nordisk says government-insured patients may use its self-pay pharmacy if they pay cash (NovoCare), and LillyDirect's self-pay terms require only that you pay cash and don't bill a government program. On Medicare, check the Medicare GLP-1 Bridge first.
The prescription has to go to the right pharmacy. Manufacturer pricing applies only when the prescription goes to that program's pharmacy. A retail chain won't honor the program price.
Promotional pricing expires. Introductory rates have fixed end dates and have changed several times since 2025. Check the official page before you budget around a number.
A denial is often a formulary preference, not a flat refusal. Many plans cover one of the two drugs cleanly and require an exception for the other. Ask which one your plan prefers before the prescription is written; it saves weeks. What the coverage criteria usually look like.

For people whose commercial plan covers Wegovy or Zepbound for weight management, or who win that coverage on appeal. Nothing else comes close: the manufacturer card takes the copay to as little as $25 a month, and you get the FDA-approved drug the trials tested. The limits are all about eligibility. The plan has to cover the drug, which usually means a prior authorization with BMI, related conditions and earlier weight-loss attempts on file. Medicare and Medicaid enrollees generally can't use manufacturer savings cards. Coverage can also end when you change jobs or the plan year turns over. If your claim was refused, start with what to do when insurance won't cover it. Last known price, Oct 7, 2026 — may have changed · confirm on LillyDirect
For people with no coverage for weight-loss drugs who want tirzepatide at the lowest compounded price we track; the figures follow the billing view you choose, the price includes the prescriber visit, and some providers make every payment non-refundable. Compounded tirzepatide has no FDA approval and was not the product tested in SURMOUNT-1: compounded tirzepatide price and legal status.
For people without coverage who want the smallest bill. Compounded semaglutide was not the product tested in STEP 1, so it is compared on price only; it has no FDA approval, and the FDA has warned about salt forms that are not the approved ingredient: compounded semaglutide price and how to check a pharmacy.
For people with a prescription, no usable coverage and a budget that reaches about four hundred and fifty dollars a fill. You get FDA-approved Zepbound in single-dose vials (or the KwikPen at the same price), shipped by the manufacturer's pharmacy, at a price no insurer can withdraw. The figures above are for the 15 mg maintenance dose: the first year includes the titration fills at $299 (2.5 mg) and $399 (5 mg), then $449 a fill at 7.5 mg and above, provided each refill lands within 45 days of the last; a full year at the maintenance dose is $5,837. Staying at 5 mg, the first year is $5,087, or $149 per pound lost. Miss that window and it becomes $499 at 7.5 mg or $699 at 10 mg and above. Vials mean drawing up your own dose with a syringe. Payments do not count toward your deductible. Cash-pay program eligibility varies by manufacturer — check the current terms. Last known price, Oct 7, 2026 — may have changed · confirm on LillyDirect

Cost per pound depends on the dose and the trial average used. The 7.2 mg HD pen is $399 a month. Needs a prescription; no insurance involved; does not count toward a deductible. How we rank: with insurance, by whether it pays and whether you have a prescriber; without it, or if you are not sure, every route within your budget, cheapest monthly cost first by how you said you would pay: month to month by default, filtered on the real month-to-month price; if you can prepay a year, one-year plans count by their monthly equivalent and are shown with what is billed upfront and the refund terms. Each card lists only the plans that fit; equal prices are listed alphabetically. Commission does not affect the order; every price shows the date it was last confirmed.

Compounded semaglutide on a prepaid one-year plan is the lowest price we track, from $69/mo on a one-year plan, billed upfront and often non-refundable; it has no FDA approval (compounded semaglutide and tirzepatide price). For an FDA-approved drug, the cheapest self-pay start is the Wegovy pill or Foundayo from $149 a month at the starting dose, then Zepbound vials from $299 through LillyDirect. On Medicare, the GLP-1 Bridge is $50 a month if you qualify. Price checked Oct 26, 2026 · joinem.co; Oct 26, 2026 · novocare.com; Last known price, Oct 7, 2026 — may have changed · confirm on LillyDirect; Oct 25, 2026 · medicare.gov
Without insurance, Zepbound vials are $299 at 2.5 mg, $399 at 5 mg and $449 from 7.5 mg through LillyDirect; Wegovy pens are $349 from 1 mg to 2.4 mg and $399 at 7.2 mg through NovoCare Pharmacy, and the Wegovy pill $149–299 a month. Zepbound's larger average loss in the trials narrows the gap per pound lost; the route table has both. Not under the regular Part D benefit, but the temporary Medicare GLP-1 Bridge gives eligible Part D enrollees Wegovy, Zepbound KwikPen or Foundayo for a $50 monthly copay, and manufacturer savings cards never apply to Medicare. It depends on the dose you end up on. For the first year from a 230 lb start, on trial averages for the same dose that is priced: Zepbound through LillyDirect is $126 per pound at 15 mg, $132 at 10 mg and $149 at 5 mg; Wegovy 2.4 mg through NovoCare is $129. The difference is the denominator: tirzepatide 15 mg averaged about 19.5% at week 52 in SURMOUNT-1 and 5 mg about 14.8%, against about 14.3% for semaglutide 2.4 mg in STEP 1 (all estimated from the trial curves), and the head-to-head SURMOUNT-5 trial found tirzepatide ahead. Averages hide a wide spread, and some people do better on semaglutide. Not for the $25 price. That copay needs commercial insurance that covers the drug. Without coverage, both manufacturers point you to their self-pay pharmacies instead, which is the LillyDirect and NovoCare rows above. Anyone on a government plan is excluded from the cards entirely. If a commercial plan refused your claim, an appeal may be worth more than any cash route: what to do when insurance won't cover Wegovy or Zepbound. Last known price, Oct 7, 2026 — may have changed · confirm on LillyDirect
About $7–10 per pound with insurance and a savings card and $126–149 for brand-name self-pay over the first year from the first dose, from each dose's average week-52 loss in SURMOUNT-1 and STEP 1, whose participants started around 230 lb. There is no per-pound figure for compounded semaglutide or tirzepatide: no compounded product was in those trials. Compounded prices start at $69 a month on a one-year plan (billed $828/yr upfront, non-refundable), or $79 a month for the first 4 weeks, and a year costs $828–$4,788 from the cheapest one-year plan to the dearest month-to-month price. Your own figure depends on your starting weight and how you respond: cost per pound falls as starting weight rises, because the same percentage loss is more pounds for the same price. It also changes with how long you stay on treatment, since most of the loss comes in the first year while the monthly price continues. Run it for your own weight and months. The first year counts every fill bought from the first dose over 52 weeks: the titration fills at their own prices, 28-day fills (13 in a year), and a first-fill promotion only where the maker applies it automatically. A compounded plan's year is its monthly charges (a posted first-month price included, a welcome-code price not) or, under Annual prepay, its one-year bill. Cost per pound divides a brand row's first year by the pounds lost at week 52 on that same dose, read from the published trial curve: 19.5% for tirzepatide 15 mg in SURMOUNT-1 (−20.9% at 72 weeks) and 14.3% for semaglutide 2.4 mg in STEP 1 (−14.9% at 68 weeks), on the treatment-regimen estimand (everyone randomized), as the prescribing information reports. Both trials added diet and activity counselling, enrolled adults without diabetes, and report averages across people whose results ranged widely. Visit fees, lab work and supplies are not included unless the provider's price includes them. This is a budgeting tool, not a prediction and not medical advice; whether to take a GLP-1, and which one, is a decision for you and a licensed clinician.
