There are five ways to pay for these drugs and they differ by more than a thousand dollars a month. Which ones you are eligible for depends on three things. Answer them and this ranks your options.
Manufacturer self-pay does not touch your deductible. Paying cash through NovoCare or LillyDirect is often the cheapest monthly number, but none of it counts toward your deductible or out-of-pocket maximum. If you have a high-deductible plan and expect other medical costs this year, running it through insurance can be cheaper overall even when the monthly figure looks worse.
Government beneficiaries are excluded from manufacturer savings. Medicare, Medicaid, TRICARE and VA patients cannot use manufacturer cash-discount programmes, even if they want to pay cash. This is a federal rule, not a company policy, and there is no way around it.
The prescription has to go to the right pharmacy. Manufacturer direct pricing only applies if the prescription is sent to that specific pharmacy. You cannot take the script to a retail chain and ask for the programme price.
Promotional pricing expires. Introductory rates on these programmes have fixed end dates and have changed several times since 2025. Verify the current number on the official page before you build a budget on it.
A denial is often a formulary preference rather than a flat refusal — many plans cover one of the two drugs cleanly and require an exception process for the other. Asking which one your plan prefers, before the prescription is written, saves weeks. What the coverage criteria usually look like.
